What is OCD therapy?

OCD therapy primarily uses exposure and response prevention (ERP), a specialized form of CBT therapy. ERP involves gradually facing situations that trigger obsessions while resisting the compulsive response. Over time, the anxiety decreases naturally and the cycle weakens. Treatment is collaborative and paced so you are never forced into something you are not ready for.

What is ERP therapy?

ERP stands for exposure and response prevention. It is the gold-standard treatment for OCD, with the strongest evidence base of any therapeutic approach for the condition. You work with your therapist to build a fear hierarchy, then gradually practice facing triggers at a pace you control while learning to sit with discomfort instead of performing compulsions. Most people notice meaningful change within weeks of consistent practice.

What kind of therapist is best for OCD?

Look for a therapist specifically trained in ERP, not just someone who lists OCD as a specialty. Ask directly: what techniques do you use for OCD, what percentage of your caseload involves OCD, and are you comfortable doing exposures outside the office if needed. A therapist who cannot clearly explain their ERP training or approach may not be the right fit for OCD specifically.

How do I know if my therapist is trained in ERP?

Ask them. A trained ERP therapist will be able to explain the method clearly, describe how they build a fear hierarchy, and tell you about their training background. Red flags include vagueness about techniques, relying only on talk therapy or relaxation exercises for OCD, or suggesting that understanding the root cause of obsessions will make them stop. ERP requires specialized training beyond general graduate education.

How long does OCD treatment take?

Many people see noticeable improvement within 12 to 20 sessions of consistent ERP practice. Severity, co-occurring conditions, and how regularly you practice between sessions all affect timelines. Treatment typically includes relapse-prevention planning so you have tools for flare-ups after the main course ends.

Can OCD therapy be done online?

Yes. Research supports virtual ERP as effective for OCD, and many therapists deliver it remotely. Online sessions can actually be helpful for certain exposures since you are already in your home environment where many triggers occur. Make sure your setup allows privacy and that your therapist has experience adapting ERP exercises for virtual delivery.

Does OCD require medication?

Not always. ERP alone is effective for many people. However, some benefit from combining therapy with medication, typically SSRIs prescribed at higher doses than for depression. Your therapist can coordinate with a physician or psychiatrist to discuss whether medication might support your progress, especially if symptoms are severe or if ERP feels too difficult to engage with initially.

What are the different types of OCD?

OCD can present in many forms: contamination fears, checking behaviors, intrusive thoughts (harm, sexual, religious), symmetry and ordering, hoarding, and relationship OCD, among others. The common thread is unwanted obsessions that drive compulsive behaviors or mental rituals. ERP is effective across all subtypes.
