What is CBT therapy?

Cognitive behavioural therapy (CBT) is a structured, time-limited approach that teaches practical skills to change unhelpful thought patterns and behaviors. You set clear goals with your therapist, learn tools like cognitive reappraisal and behavioral experiments, and practice small steps between sessions. It is one of the most studied therapies in psychology, with strong evidence for anxiety, depression, insomnia, OCD, and many other concerns.

What happens in a CBT session?

Sessions follow a structured format: you set an agenda, review homework from the previous week, work on a specific skill or pattern, and plan practice for the coming days. Your therapist may use worksheets, thought records, or behavioral experiments to make the work concrete. Most sessions are 50 minutes, and the structure is what makes CBT feel different from open-ended talk therapy.

What is CBT best for?

CBT has the strongest evidence base for anxiety disorders, depression, insomnia, and OCD. It is also used for PTSD, eating disorders, chronic pain, and anger management. For anxiety and panic, start with anxiety therapists. For persistent low mood, depression counselling is a good filter. For obsessions and compulsions, OCD therapy providers often use ERP, a CBT-based method. CBT can be adapted to most concerns where thoughts and behaviors are maintaining the problem.

How long does CBT take?

Many courses run 8 to 16 sessions, with booster visits to consolidate change. Some focused protocols (like CBT for insomnia) can work in as few as 4 to 6 sessions. Timelines depend on the concern, severity, and how consistently you practice between visits. For more specialized contexts like eating disorder therapist treatment, CBT may be adapted and combined with additional supports over a longer period.

What is the difference between CBT and DBT?

CBT focuses on identifying and changing unhelpful thoughts and behaviors. DBT therapy builds on CBT but adds structured skills for emotion regulation, distress tolerance, and interpersonal effectiveness. DBT was originally developed for borderline personality disorder and is often a better fit when emotional intensity, self-harm, or relationship instability are central. ACT therapy is another related approach that emphasizes acceptance and values-based action over thought change.

How much does CBT cost in Canada?

Private CBT sessions typically cost $150 to $250 per hour depending on the provider's credentials and location. Most extended health plans reimburse sessions with registered psychologists, social workers, or psychotherapists. Some hospitals and community health centres offer publicly funded CBT programs, often with waitlists. Online CBT programs (guided or self-directed) can also be a lower-cost option for mild to moderate concerns.

Can I do CBT on my own?

Self-guided CBT workbooks and apps can help with mild symptoms, especially for anxiety and insomnia. However, working with a trained therapist is more effective for moderate to severe concerns, because they can tailor the approach, catch blind spots, and adjust the plan when you get stuck. Many people start with a therapist to learn the core skills, then maintain gains independently with occasional check-ins.

What is CBT for insomnia?

CBT-I (cognitive behavioural therapy for insomnia) is a structured program that targets the thoughts and behaviors keeping you awake. It typically involves sleep restriction, stimulus control, sleep hygiene, and cognitive restructuring. Most protocols run 4 to 6 sessions. Research consistently shows CBT-I is more effective than sleep medication for long-term results, and it is recommended as the first-line treatment for chronic insomnia by most clinical guidelines.
